The comprehensive long term care system begins with NY Connects: Choices for Long Term Care (NY Connects), a local program that operates in most counties of the State. NY Connects provides easy access to information and assistance for people who are exploring long term care options or who are already receiving a long term care service but would like more information. In Clinton County the Office for the Aging, Department of Social Services and the Department of Health work together to bring you Clinton County’s answer to NY Connects.
